Not cutting Second and Third Pass in LIne

I have been cutting a Square with 3 Pass and the Second and Third pass are outside the Square.

I tightened the Belt but that did not help, Cleaned the Lens they look great,
Cleaned the Top Camera Lens , I am at a loss. ?
